Kempen Ten Smiles is run in the Lilse Bergen, in the middle of the Kempen countryside, and consists of one large loop that runs for more than ninety-five percent over unpaved paths. There is a choice between five and ten miles. The start is organised in waves spread across the early afternoon, and everyone runs with timing. Entry is open from twelve years of age. Beside the course lies an event village that opens before the first start, with a bar, terrace, food trucks, a DJ and children's activities, so supporters can make an afternoon of it. That combination of a sandy, wooded course and a distinctly festive setting is what characterises the race.
